That is a fine answer. We would rather have the report than the right label, and choosing this does not weaken it.

What this is, and what it is not

  • TheraLume is not an independent review body. Nothing here is an investigation, a finding, or an adjudication.
  • Choosing a category sorts a report. It does not decide legal responsibility for anything, and neither does sending one.
  • You can complain elsewhere at the same time, or instead. If a regulated clinician is involved you can complain to their regulatory college directly — you do not need us. A privacy complaint can go to your provincial or federal privacy regulator, and anything you believe is a crime can go to the police.
  • We do not forward this report to a clinician, a college, a regulator, an insurer or the police automatically. Nothing leaves TheraLume because you pressed send.

2. Tell us what happened.

An approximate answer is fine. Do not invent a date you are not sure of.

Clinical facts about the incident itself are allowed here — what was said, what was done, what happened to you. You do not need to give a full medical history, and you do not need to send a chart or a record. Write only what the incident needs.

There is nowhere here to add a screenshot or a document, and that is deliberate: the secure way of accepting them is not in service, so we are not going to ask you to hand them over into something we cannot stand behind.

Saying what you want does not mean it will happen. It tells us what you are asking for.

Who can read this

TheraLume staff who handle concern reports. Nobody else is sent a copy. The clinician you are writing about is not shown this report by the act of sending it.

There is no reviewer on duty for these reports today. Sending this stores it. It does not alert anybody, and nobody is waiting to read it.

Limits of confidentiality

This is not confidential the way a therapy session is. Staff who handle reports can read it, and we may have to disclose it if the law requires that of us — for example under a court order, or where a child or someone at serious risk is involved. Write what you need to write, knowing that.

Complaining somewhere independent

  • A regulated clinician's regulatory college takes complaints about that clinician directly. You do not need TheraLume's permission, and you do not need to report here first.
  • Your provincial or territorial privacy regulator, or the federal one, takes privacy complaints directly.
  • The police take reports of anything you believe is a crime. Call 911 if it is happening now or someone is in danger.
